The first step in the referral process is to fill out the basic Care Assistance Questionnaire linked at the bottom of this page. Mike will be attending our in-person monthly meetups to answer questions, or you can reach out to Brandi to get referral information.
The consultations are free. Only if you decide to purchase any kind of plan, or file legal paperwork that requires fees, will you pay out of pocket.
